| The Neurosurgery Group of
Western Pennsylvania is a physician referred specialty practice.
Your physician must make a referral to the practice.
Your insurance company must also approve the referral.

New
Patients
- Once you have a referral, call 412-682-5900.
- After scheduling your appointment, you will receive a
confirmation via mail or e-mail, whichever you prefer.
- Download the patient
information forms listed below, fill them out
as completely and legibly as possible, and
bring them with you to your appointment. (You will need to view and print the forms.)
Ask that the forms be mailed to you if you cannot download
or print them.
- Directions to our offices can be located on our Web site.
- You will need the ACTUAL FILMS of your recent X-rays,
MRIs, CT scans, or myelograms. Request copies
of the films from the facility where they were
performed and hand carry them to your appointment - DO
NOT have them mailed. ()
- Bring any written records, medical records, test
reports that you have available.

Follow-up Visits
- The referral for your initial consult will usually cover
follow-up visits for 60 days. Check with your insurance
plan.
- Visits following surgery are covered within the 90 day "global
period" for surgical procedures (the "global
period" for
some minor procedures is shorter).
- Call 412-682-5900.
- After scheduling your appointment, you will receive a
confirmation via mail or e-mail, whichever you
prefer.
- ALWAYS bring ALL of your ACTUAL FILMS of your X-rays,
MRIs, CT scans, or myelograms, even if your
surgeon has reviewed them previously – they may need to be
reviewed again. If you have had a new study, bring
copies of BOTH the new study and previous studies
for comparison. Request copies of the films from the facility
where they were performed and hand carry them to your appointment
– DO NOT have them mailed. ()

Previous Patients
If you were released from our care, and are returning
for evaluation of the same or different problem, then you
are considered a "previous patient." You will need
a new referral. You will also need to update your registration
forms. If you have not had any new studies performed since
your last visit, then you may be scheduled with the , who will assess your situation, and in consultation
with your surgeon, determine the need for medication and/or
imaging studies. Your next return visit will be scheduled
with the surgeon for review of those studies. |
